The Slough Together Walk – 2010 is a circular walk of 5 miles.
PLEASE PRINT CLEARLY YOUR NAME, ADDRESS & POSTCODE. WE ARE REQUIRED BY LAW TO PRODUCE THESE DETAILS IF ASKED.
GIFT AID: For every £1 you donate to charity the taxman will add another 28pence. To qualify you must pay Income or Capital Gains tax, an amount at least equal to the amount of gift aid being claimed in the current tax year. Please enter a tick in the Gift Aid column if you qualify and add your regular signature.
Information for entrants:
All sponsorship money along with this sponsorship form should be sent to The Treasurer, The Rotary Club of Slough, Abbots Mead House, Farnham Royal, Bucks, SL2 3SF by 31st August, 2010. Cheques should be made payable to The Rotary Club of Slough.
IMPORTANT NOTICE: THIS SPONSORSHIP FORM IS YOUR PROOF OF ENTRY. PLEASE BRING THE COMPLETED FORM WITH YOU ON THE DAY & PRESENT IT AT THE START RECEPTION. ON COMPLETION OF THE WALK YOU WILL BE ABLE TO COLLECT YOUR SIGNED COPY AT THE FINISH RECEPTION TO LET YOUR SPONSORS KNOW OF YOUR SUCCESS.
